Patient Identification

We can provide Virtual Care Services to support your patients through

  1. Detect and Prevent
    Proactive identification of 'at-risk' patient groups. Predictive modelling & risk stratification of populations at risk of developing an undiagnosed disease
  2. A&E Prevention / Triage
    Direct engagement with local A&E Teams for patients requiring additional levels of interventions to be managed at home
  3. Supporting Hospital Discharge
    Dedicated Clinical Engagement personnel to work with you to identify the most suitable patients for Virtual Care and enable earlier hospital discharge.

Risk Stratification

Patients will be placed into care streams dependant on clinical and social need

LOW: Usual GP Care for low risk patients

MEDIUM - Self-managed support enabled through virtual technology, telephone support and daily observation monitoring

HIGH: Virtual Care F2F nursing interventions for patients with complex needs. Increased monitoring and onward referral to allied professionals as needed following daily MDT review

Tech Enabled

  • IQVIA will provide easy-to-use technologies to support patient’s with their virtual care.
  • Mobile/desktop apps will allow for virtual monitoring of key health indicators, coupled with wearable devices when appropriate
  • Remote monitoring via telephone will also be available to those with limited internet connectivity

Care Delivery

  • A team of IQVIA nurses / clinical pharmacists will review patients daily and support patients in line with current treatment pathways including self-management initiatives, medication management or the provision of more acute nursing interventions.
  • Patient care will  be delivered either virtually or in-person, dependant on clinical pathway of the patient.

Data Integration

  • IQVIA will integrate clinical patient outcomes and reporting with existing electronic medical record systems (e.g. EMIS or SystmOne)
  • All patient data is reported to the NHS & the health care professional, giving a holistic and collaborative approach to patient care.
